Core Daily Care
Personal hygiene: Assisting with bathing, showering, grooming, dressing, and oral care.
Toileting and mobility: Helping with bathroom need
Medication support: Assist giving medication by physically putting medication in client mouth [avoiding falling out] & providing a straw to prevent sitting it back into the cup.
Household and Support Tasks
Meal preparation: Assist client and or housekeeper to prepare nutritious meals and helping with feeding if needed.
Light housekeeping: Handling tidying, washing dishes, and keeping the living space clean. [Family Room]
Monitoring and Communication
Health observations: Tracking physical or behavioral shifts
(like changes in mood, appetite, or mobility) and reporting them immediately to the family.
Companionship: Offering meaningful social interaction and emotional reassurance to prevent isolation. A MUST

The Health Care Provider charting in a home care setting must record daily care activities, vital signs, changes in health status, and incidents. This documentation ensures safe care, tracks client progress, and provides a legal record for the health care team. Record personal care given, like bathing, grooming, and dressing. Note meal and fluid intake amounts. Track bowel and bladder habits or changes. Log help given with walking or moving. Write down vital signs if trained and asked to do so. Note any new pain, skin redness, or wounds. Report changes in mood or memory. Track signs of unusual tiredness or weakness. Write down any falls, slips, or accidents right away. Note home safety hazards, like broken equipment. Record refusal of care & Notify the Family immediately.
